An on-line fee-based physician profiling system is available for credentialing entities. A log-in ID and password will be required to access the system. In order to receive your log-in ID and password, you will have to submit the following forms, completed in their entirety, and the required payment. A payment of $500 is required for everyone except hospitals credentialing ten physicians or less; hospitals credentialing less than ten physicians are required to pay $250. This fee will provide you with unlimited access for one year, at which time you may renew your subscription. The physician profile system will be updated nightly (except weekends and state holidays) to allow for up-to-date information. | |
