Permanent Medical Reinstatement

For physicians (MD), osteopaths (DO), podiatrists (DPM) and physician assistants (PA) seeking reinstatement of a Mississippi license that has lapsed.

At a glance: A license that has been expired for more than one year is reinstated, not renewed. Reinstatement applications are submitted online through MELS (Medical Enforcement & Licensure System). A license expired for one year or less is renewed late instead.

Fee: $250 application fee, non-refundable, credited toward the back fees owed after Board approval. View full fee schedule.

License validity: If the reinstatement is approved by the Board on or before March 31, the license expires June 30 of the current year. If approved on or after April 1, the license expires June 30 of the following year.

Important rules before you start

Reinstatement Eligibility. A licensee who allows a license to lapse by failing to renew may be reinstated by the Board upon a satisfactory explanation for the failure to renew, completion of a reinstatement application, and payment of the arrearages of renewal fees and applicable fines (see Reinstatement Fees below).

Inactive Practice Requirement. Prior to the reinstatement of a license, any licensee who has not actively practiced for a three (3) year period shall be required to participate in a Board-approved assessment program, clinical skills assessment program or re-entry program to assure post-licensure competency. A licensee shall be deemed to have not “actively” practiced medicine if, during said three (3) year period, he or she has not treated any patients for remuneration, other than friends and family.

Expired Five Years or More. If a license has lapsed for five (5) years or more, the applicant is required to submit fingerprints for a criminal history records check. The Board may, at its discretion, require fingerprints for applicants whose license has lapsed less than five (5) years.

Online Application Timeline. Your online application stays active for 90 days from the date the Board receives each verification from an outside institution. Each new verification received resets the 90-day window, up to a maximum of 365 days total from the date of submission. If the application is not completed and the license is not issued within 365 days, the application will be considered null and void, and a new application must be submitted along with all required fees and certifications. The 365-day time limit cannot be waived under any circumstances.

Application Progress. Once submitted, applicants may track progress through MELS (Medical Enforcement & Licensure System). Upon submission, the applicant shall promptly provide all information deemed necessary by the Board to process the application.

Incomplete Application Fee. A one-time $50.00 fee will be assessed if MSBML staff discover during application review that the application is not complete. This includes, but is not limited to: any public record not disclosed on the application (for example, prior name changes or prior arrests), or unreported segments of graduate training (for example, postgraduate training completed in part at one facility and in part at another must list both facilities and the time spent at each).

Refund Policy. No refunds will be issued under any circumstances.

How to Apply

  1. Log in to MELS

    Sign in to MELS (Medical Enforcement & Licensure System) and start the reinstatement application. All applications are submitted online.

  2. Request your primary source verifications

    Send the Activity, State Medical Board Licensure and Staff Membership certification forms to each institution and request that they return the completed form directly to MSBML. DPM applicants also request their CPME and NBPME records.

  3. Complete and submit the online application

    Complete each section of the online application in MELS, upload your applicant-side documents, and pay the $250 application fee.

  4. Mail your Affidavit and physical items

    Mail or hand-deliver the ORIGINAL notarized Affidavit and Perpetual Release of Information (with wallet-sized photo attached) and, if required, completed fingerprint cards to the Board. LiveScan appointments are scheduled by the Board.

  5. Track your application and pay back fees after approval

    Track progress in MELS (Medical Enforcement & Licensure System). Each new verification received resets the 90-day window; respond promptly to any Board requests. If the Board approves the application, pay the back fees for up to the previous five renewal years plus the current year’s renewal fee.

Items Submitted by the Applicant

Submit these items upon completion of the online application and initial reinstatement fees. All documents received become the property of the Board and will not be returned. Original notarized documents must be mailed or hand-delivered to the Board; see the Notary Guidelines and Certification Form. PDFA. Affidavit and Perpetual Release of InformationSigned and notarized. The ORIGINAL must be mailed or hand-delivered to the Board. A copy must be included with each hospital privilege form sent to a hospital.
BMississippi Practice PlansAn explanation of your practice plans in the State of Mississippi should the application for reinstatement be approved. May be emailed to certification@msbml.ms.gov.
CPhotograph (physical and electronic)Passport-type photo taken within the last 60 days, in two forms: (1) physical wallet-sized print attached to the Affidavit, and (2) electronic copy uploaded via MELS. Informal snapshots will not be accepted. See Photograph Guidelines.
PDFD. Applicant Supplemental Affidavit and Narrative FormRequired if applicable. Submit with the supporting documentation if requested by the Board through the application process.
EApplicant FingerprintsRequired if the license has lapsed five years or more; the Board may require them for a shorter lapse. Two options: (1) FD-258 fingerprint cards mailed by the Board to your address, or obtained from a local law enforcement agency or fingerprinting service provider, completed and returned to the Board; or (2) LiveScan at MSBML, scheduled by the Board. FReinstatement FeesUpon the Board’s approval of the application, pay the back fees for up to the previous five renewal years, plus the current year’s renewal fee. See Reinstatement Fees below for the full breakdown.

Verifications Requested by the Applicant (Primary Source)

MSBML is a primary source verification board. The following verifications must be sent directly from the issuing institution to the Board. You are responsible for sending each form to the appropriate institution and requesting that they return it directly to MSBML.

DPM applicants only

The following two verifications apply only to applicants seeking reinstatement of a DPM (podiatry) license.

  • Residency Training Verification: Request from The Council on Podiatric Medical Education (CPME) at cpme.org.
  • NBPME Certification: If you are a Diplomate of the National Board of Podiatric Medical Examiners, request a transcript to be sent to the Board at apmle.com.

Reinstatement Fees

$250 application fee, non-refundable, paid through MELS. If the application is approved by the Board, the applicant will be required to pay back fees for up to the previous five renewal years, plus the current year’s renewal fee. These arrearages include a fine of $25.00 plus $5.00 for each month the renewal remains delinquent. The $250 application fee will be credited toward the amount due. A one-time $50.00 incomplete-application fee may be assessed if MSBML staff determine during review that the application is not complete.
